PAWNEE MENTAL HEALTH SERVICES JOB DESCRIPTION Title of Position: Case Manager/ Waiver FLSA Status: Non-exempt Program: Youth Rehabilitation Services (YRS) Supervisor: Youth Rehabilitation Services Supervisor Supervises: Not Applicable Minimum Qualifications: Bachelors degree in a human service field or equivalent work experience, or a combination of work experience in the human services field and education with one year of experience substituting for one year of education required. Knowledge of the needs and challenges of youth who experience serious emotional disturbance and their families preferred. Strong oral and written communication skills using the English language required. Basic personal computer skills which enable the entry, retrieval and use of electronic data required; experience with Microsoft Office software preferred. A valid driver's license and automobile liability insurance maintained in accordance with Pawnee policy and procedure is required. Results of criminal and all other applicable background checks must meet the approval of Pawnee Mental Health Services. Essential Job Functions of Case Manager: * Engage clients at home, school and in the community in a supportive and helping relationship by eliciting and facilitating their/their family's choices in meeting their basic needs. * Perform on-going strengths assessment services and activities which engage the individual/family as an active participant and addresses their strengths, wants, needs, preferences, etc. * With input from the client, their family and other members of the treatment team develop and document treatment goals and plan related services and activities. Ensure services are appropriately authorized, then monitor and document progress and concerns. * Assist client/family in maintaining benefits needed to facilitate the rehabilitation process. * Review client incidents with supervisor and submit written report as outlined in policy and procedure. * Advocate for client rights and community integration. Preserve dignity and confidentiality of each person on caseload. * Perform verbal and, if necessary, physical behavior management interventions as outlined in the treatment plan. * Within scope of knowledge, provide resources and education to the client and their family about mental illness or serious emotional disturbances upon request. * Meet administrative and clinical documentation requirements both in terms of content and timeliness. * Meet direct service expectations in terms of type and quantity of services provided as prescribed by management. * Drive to community settings for the delivery of services and related program needs. This includes transporting the client in a personal or company vehicle. * Attend and actively participate in clinical supervision and program staffing. Provide input to supervisors and other treatment team members regarding clinical and program issues. * Enter and maintain clinical schedule in ClaimTrak scheduler. * Maintain and advance knowledge and skills related specifically to position or to field of work through regular review of pertinent materials or participation in training events as approved by supervisor. * Adhere to the standards, policies and instructions established by PMHS Policy and Procedure and any applicable ethical codes within field of employment. * Review and comply with the Pawnee Code of Conduct outlined in the Employee Handbook. Essential Job Functions of Waiver: 1. Comply with requirements of the State of Kansas Home and Community Based Services (HCBS) severely and Emotionally Disturbed (SED) waiver. 2. Assist in the assessment of HCBS SED waiver referrals to determine the level of waiver services needed to prevent a higher, more restrictive, level of care. 3. Establish an initial written Plan of Care (POC) that includes access to informal community based resources, the child and family's strengths and needs, treatment goals, objectives and actions steps, and a crisis plan that will help the child succeed in the community. Update the POC on the family choice date annually. 4. Monitor current services, child and family/caretaker needs and service outcomes while the child is enrolled in the HCBS SED waiver to determine if services received are as prescribed in the current treatment plan/Plan of Care. 5. Promote collaboration among family, caretakers, service providers, educators and community resources to ensure that appropriate and effective services are delivered to the child and family/caretaker. 6. Ensure a Child Behavior Checklist (CBCL) has been completed and updated semi-annually with the scoring information being documented in the clinical chart. 7. Utilize the State of Kansas data entry system to track and submit required HCBS SED waiver data. 8. Ensure the tasks and documentation outlined in each client's Plan of Care are completed so service authorizations are maintained. Track these activities on the waiver spreadsheet. 9. Review Automated Information Mapping System (AIMS) data and collaborate with Youth Rehabilitation Services for completion and submission to the State. 10. Provide support and consultation to staff, community and clients about the process for HCBS SED waiver application and eligibility. 11. Attend clinical staff meetings to monitor and track the child's progress with the Plan of Care and additional resources or services that are necessary. . 12. Maintain and advance knowledge and skills related specifically to position or to field of work through regular review of pertinent materials or participation in training events as approved by supervisor. 13. Other Job Functions: * Perform other reasonably related duties as assigned by immediate supervisor or other management personnel. * Maintain all waiver specific records for all EYRS clients on the SED Waiver * Ensure all POCs are completed and entered into Pawnee's EHR and the State's system on time * Check initial paperwork for accuracy * Train staff on waiver process as appropriate * Submit state paperwork on time QUALIFICATION REQUIREMENTS: To perform this job satisfactorily, an individual must be able to perform each essential duty satisfactorily.
