Job Description:
Angels Care Home Health, a distinguished provider in home health

compassionate and skilled Occupational Therapist to join our team.
This position will play a critical role in helping patients regain,
develop, and build skills that are important for independent
functioning, health, and well-being. Our Occupational Therapists
are dedicated professionals who work closely with patients in their
homes to deliver personalized therapy and promote optimal wellness
through tailored treatment plans. Evaluate patients’ conditions and
develop appropriate occupational therapy treatment plans. Help
patients develop, recover, improve, as well as maintain the skills
needed for daily living and working. Record patient progress and
modify treatment plans accordingly. Educate patients’ family
members and caregivers on patient care. Ensure compliance with all
legal, regulatory, and clinical policies and guidelines. Assess
home environments of patients and recommend changes to support
patient needs and safety. Collaborate with other healthcare
professionals to understand and meet patient care needs. Strong
analytical skills to assess patient conditions and interpret
findings. Ability to develop and follow through with a
comprehensive treatment plan. Communication skills to effectively
discuss progress and treatment plans with patients, family, and
other healthcare personnel. Patience and compassion for working
with patients who have severe limitations and challenges. Ability
to manage time effectively to ensure patient contact hours are met
and documentation is completed promptly. Medical Insurance Dental
Insurance Vision Insurance 401k Current Occupational Therapist
license for the practicing state. NBCOT (National Board for
Certification in Occupational Therapy) certification.
